How Complaints Are Processed

Complete This Form
Provide your information (Complainant), the respondent's information, and any witnesses who have direct knowledge of the alleged conduct.

Identify the Ethical Violations
List each Ethical Principle and Section you believe was violated, with a brief description of the behavior in question.

Attach Supporting Evidence
Include any documentation that supports your allegations. Witness testimony must be notarized.

Sign & Submit
Your electronic signature authorizes ACBHC to release the information necessary to investigate this complaint. The complaint will not be processed without it.

Await Ethics Committee Review
If the respondent is certified through ACBHC, your complaint will be forwarded to the Ethics Committee and appropriate procedures will be followed. You will receive further notification.
